PROTECTION FROM ABUSE (PFA) FORMS
Below is a listing of and links to blank forms that are necessary in filing for Protection From Abuse (PFA). 
They are separated into two (2) categories:
REQUIRED FORMS
or
SITUATIONAL FORMS
The forms listed under "Required Forms"
MUST be submitted in order for your filing
to be accepted by the Court.
 
The forms listed under "Situational Forms"
should only be filed if your circumstances
match those described.

IF you are not able to determine which forms you need to file, please review UNDERSTANDING THE PFA PROCESS, visit the Family Court Resource Center in your County, or consider contacting an attorney.  ALL Pro Se litigants are encouraged to review UNDERSTANDING THE PFA PROCESS while preparing their filing.


REQUIRED FORMS:
» Petition for Protection from Abuse (Form 450)
» Information Sheet (Form 240)
   
SITUATIONAL FORMS (Please review all forms and scenarios):
» Affidavit of Unknown Address (Form 241)
  This form should be filed if you do not know where the Respondent(s) live
» Affidavit for Emergency Expedited Hearing (Form 656)
  This form is filed to request that the Court place your pleading for an Order for Protection from Abuse on an expedited calendar
» Affidavit for PFA Exparte Order (Form 654)
 
· This form is filed to request an immediate hearing with the Court (occurring either the same day or the following day)
· If an Ex Parte order is granted, it will remain in effect until the date o your full PFA hearing
